Submit a Referral
Send us client information via our secure referral form, email, or phone. Include support needs, interests, and any preferences.
Initial Assessment
We review the referral and schedule an intake meeting with the client and family to understand their goals and preferences.
Matching Process
Our team identifies compatible supportive roommates based on personality, interests, location, and support requirements.
Introductions & Meetings
We facilitate multiple meet-and-greet sessions between the client and potential roommates before any decisions are made.
Transition Support
Once matched, we coordinate the transition with you, the client, and the roommate to ensure a smooth move-in process.
Ongoing Coordination
We provide regular updates and remain a resource for you throughout the placement. IPP meetings are always welcomed.
